Case Management is about people helping people. Our case managers work aggressively with the individual, families, guardians and natural supports to access whatever assistance or supports that are needed to help people create lives that are meaningful and fulfilling.

We provide you with information about all of your options and assist you to develop a plan of service that helps attain your values and goals. Our staff of Case Managers have over 30 years of combined experience and are committed to working together with you. We realize life can be hectic. And because of our commitment, flexible hours are available.

Our Services
Information and Referral—Information about MR/DD services or services provided by other agencies. You do not have to be eligible for Case Management to get information and referral services.
Service Placement—Case Managers help people find the right school, the right job, or the right place to live.
Service Coordination—Case Managers help coordinate services from multiple providers.
Advocacy—An advocate is defined as “one who pleads on behalf of another.” In other words, an advocate is a person who will help another person with a problem they cannot handle alone. Case Managers protect the rights of people with MR/DD.
Service Monitoring—Case Managers check to see that a person with MR/DD is getting help. They may visit a person’s home, school program, or workplace to make sure the individual is receiving the services they desire.
Major Unusual Incident Investigation/Reporting—Any event which could or does affect the quality of life of a person—such as abuse or an unexpected serious illness is a major incident. Whenever a major incident occurs in the life of a person with MR/DD, a Case Manager is responsible for reporting the incident to the designated authorities.

Eligibility
Who is eligible for Case Management Services?
Anyone in Kansas who has MR/DD may be eligible. Eligibility is determined by your local Community Developmental Disability Organization. The process can be confusing. Choices Network Inc. can assist you in gathering the information needed to determine eligibility. Please call our office for more information on eligibility.
