Case Management is about people helping people. Our case managers work aggressively with the individual, families, guardians and natural supports to access whatever assistance or supports that are needed to help people create lives that are meaningful and fulfilling.In all areas of our business, we are committed to working together with individuals and families to provide high quality, responsive services. Our mission is to coordinate services to individuals that promote mental and physical well-being, personal dignity and independence.
We will provide you with information about all of your options and assist you to develop a plan of service that helps attain your values and goals. Our staff of Case Managers has over 30 years of combined experience and is committed to working together with you. We realize life can be hectic. Flexible hours are available.
Who is eligible for Case Management Services?
Anyone in Kansas who has MR/DD may be eligible. Eligibility is determined by your local Community Developmental Disability Organization. The Process can be confusing. Choices Network Inc. can assist you in gathering the information needed to determine eligibility. Please call our office for more information on eligibility.
