Personal Choice Community Based Day Service
Imagine a day service program designed specifically around what you like to do and what your support needs are. Do you want a job in the community or maybe you’d like to volunteer at your church? With Personal Choice Community Based Day Services you can dream outside the box. All of our services are uniquely designed to allow you to live in a totally integrated environment, choose how and with whom you want to spend time with, and remain connected to your family and friends. It’s about You, your family, and what is important to you.

Eligibility Criteria

Must have a developmental disability and be deemed eligible for HCBS services
Must be a resident of the state of Kansas
Must be 18 years of age or older
Must be willing to accept terms and conditions of the program


Your Responsibilities

Participate in an assessment to determine an individualized support plan. This plan will determine what needs and safeguards are required to ensure health and safety. Each plan shall be reviewed at least once per year.
Choose the person/s you want to hire to provide your support
Decide what special knowledge and skills, if any, the person(s) you want to hire must have
Train attendants to meet your specialized needs
Schedule, supervise and replace attendants when necessary
Follow all laws and rules on employing attendants
Complete and manage the necessary paperwork
Have emergency plan in place to deal with health and attendant care emergencies
If consumer support needs exceed the attendant support plan, request a review of the current plan
Must be out of residence an average of 5 hours per day for a total of 25 hours per week participating in activities that include work, socialization, recreation, community inclusion, environmental awareness/stimulation, and mobility training


Our Responsibilities

Review and approve your support plan
Confirm the choice of your attendants
Perform all background checks required
Provide you with forms to use for reporting the hours worked by attendants
Make payments to your attendants based on your individualized support plan
Act as the employer of record for all attendants
Withhold all federal, state and federal unemployment taxes from attendant paychecks
Pay employer taxes and provide workers compensation insurance
Advise attendants of their rights as employees
Review attendant timesheets for any comments or concerns to pass on to the Program Administrator
Review the information you send in to make sure it is complete and accurate
Provide CPR and first aid
training
Monitor quality indicators
Ensure various safeguards are in place to promote health and safety
